An anomalous origination of a coronary artery from the opposite sinus of Valsalva is one of the most common coronary artery anomalies. If it happens to be a malignant type, it can cause a disastrous accident such as sudden cardiac death. Because the treatment options and prognosis are different according to the types of the anomalies, an accurate diagnosis is critical. We report a case of a right coronary artery originating from the proximal left anterior descending artery, which was discovered incidentally while evaluating chest pain. Coronary computed tomography has played a pivotal role in determining the subtype of anomaly by observing the course of coronary arteries.
